Registration Protocol during COVID-19 Emergency

During the Covid-19 emergency your GP practice, like the rest of the NHS, is prioritising patient care. To minimise administrative work, all GP practices locally are requesting that you (and other members of your family) do not seek to re-register with another GP practice at this time. You should only ask to register with a new GP practice if:
1. You, and your family, have physically moved to a new house and are now living in a new GP practices area
2. You have a new-born baby, or a newly adopted baby or child
3. You are a first degree relative of a currently registered patient who has returned to live with their family
4. You do not currently have a GP, and need to register with one

If you have moved to a new house, but remain within your current GPs practice area, please do not re-register with a new GP practice at the present time, even if you intend to do so in the future.
Please do not seek to register with a new GP practice unless one of these four reasons apply to you.’

 

If you live in our practice area and wish to register we need certain information from you and each member of your family to be able to trace your previous medical records.

All patients are required to provide photographic proof of identity (e.g. passport, photocard driving license). In addition all patients aged 16 years or over need to also provide current proof of address (e.g. utility bill no older than 3 months).

All patients from Non-EU countries must present their passport with a valid visa.

Please register by either completing the relevant forms which can be downloaded/viewed via the links at the bottom of this page, or by calling into the surgery to collect the documents.

PLEASE NOTE: Our busiest times are before 9.30am and after 5.30pm so please try to avoid coming to the surgery to register at these times.  The surgery is closed between 1pm and 2pm every day.

Once you are registered with the practice you can also register for our Online Patient Access Service which allows you to book appointments, order repeat prescriptions and view certain aspects of your medical records.
